- Listed below are all of the documents included in the DCPS enrollment packet.
- To enroll, submit the required forms below to your child’s school.
- Families of special education students who receive funds from DCPS to attend non-public schools may return their forms to the DCPS Office of Specialized Instruction (OSI) (1200 First Street NE, 9th floor). For questions, contact OSI at (202) 673-4488.

Free and Reduced Meals (FARM) Application
- If your child does NOT attend one of the CEP schools, you must complete and submit a FARM application even if you are delclining this service.
- 2015-2016 FARM Applications will be available for families beginning July 1, 2015.
- Applications will be available in the main office of each eligible school and online in various languages after July 1, 2015.
